Requesting Records
Patients in need of physical copies of their Medical Records may obtain them as needed by contacting the Medical Records department of Moore County Hospital District at (806) 935-7171.
Medical Records can only be obtained by the actual patient or the legal guardian of the patient. You will not be able to obtain the Medical Records for individuals whom you are not legally responsible due to federal HIPAA regulations.

Birth Certificates
MCHD will provide parents of babies born with us a complimentary memento certificate with your baby’s footprints on it.
The original birth certificate will be filed at the Moore County Clerk’s office (806) 935-2009 located at the County Clerk’s office in Room 7 on the first floor of the Moore County Courthouse located 715 South Dumas Ave.
You may pick up the certified copy 2 weeks from the date of birth. The charge to pick up the certified copy of the birth certificate is $23.00 cash or debit card only. You must also show government issued identification. The form to bring to the courthouse is available below for your convenience.
You may also get more information from the County Clerk’s office by clicking HERE or you may also obtain a copy of a birth certificate of children not born at MCHD through the Texas Vital Statistics department of the Texas Department of State Health Services online by clicking HERE.

Social Security Cards

Your baby’s Social Security card should come in your mail approximately 4 to 6 weeks after the date of birth. If it does not arrive within 6 weeks, please contact 1-800-772-1213 for any and all Social Security questions.
